Group Insurance Application for Self-Paid Insured (must submit by Sep. 20, 2023)

Dear faculty and staff,

Please be informed that the additional group medical insurance for full-time colleagues (faculty and administration staff) now is open for application of self-paid items, including increasing premium for personal partial coverage and application of covering your spouse and lineal relatives. Dependents are charged on a per-person basis. Your spouse, parents and children's premiums vary depending on age and coverage. In this academic year, the insurer is now Taiwan Life Insurance. The insurance premium is different from the 2022 academic year. Please refer to the attachment: Self-Paid Insurance Application Form for more details.

If you want to apply for self-paid items, please reply by September 20, 2023, no late submission will be accepted. For those who have already purchased self-paid items in the previous academic year, the insurance will be automatically renewed, no application is needed, unless you apply for a termination or change of insured. Also, the application must be made by September 20, 2023. 

**For termination of the self-paid insurance, please reply to the email with the name of insured.
**For application of new self-paid insured, please fill in the attached Form.
